Our top priority at Christian Brothers High School is the formation of our Brothers' Boys, mind and body. Through our partnership with Campbell Clinic, we work with some of the finest athletic trainers, physical therapists, doctors, and medical personnel in the region. Dr. Fred Azar, Campbell Clinic chief of staff, oversees the team that works with our coaches and student-athletes throughout the year, and you will often see him on the sidelines or courtside! Dr. Azar serves as team doctor for the Memphis Tigers.
The following resources are here to inform you of our school's athletic policies, the necessary forms parents must complete for their sons to participate in sports at CBHS, and recommendations for students' optimal success on the field of play.

Free physicals will be offered to new and returning CBHS student-athletes at Campbell Clinic's 1400 S. Germantown Road location:
Saturdays, June 6 and 13, from 8 to 11 a.m.
Required Documentation of Medical History and Physicals
Every CBHS student-athlete — new and returning — needs a physical and a submitted set of online forms before they can participate. Here's what to do.
Step 1: Annual Physicals
Campbell Clinic physicals are held on select Saturday mornings in June at its 1400 S. Germantown Road location, on a first-come, first-served basis — no appointment needed. You may also have your son's physical performed by his primary care physician. Either option works, but the completed form must be submitted by the season deadline.
Before you go, complete the first page of the TMA/TSSAA Pre-Participation Medical Evaluation (Physical Packet).
- If your son is seen at Campbell Clinic, bring the packet with the completed first page and hand it to their staff the day of his physical.
- If you use your own physician, have them complete their portion and submit the full form by the deadline (See Step 3).
Step 2: FinalForms Account Setup
All registration, medical history, and insurance information is completed electronically through FinalForms, the same platform our athletic trainers use to manage your son's care during his time at CBHS. Creating an account ensures our staff has up-to-date emergency contacts and an accurate medical history on file before your child participates in sports.
NOTE: A parent account must be set up first, then the student's account. Follow the steps below to create you and your son's FinalForms accounts.
Step 3: Deadlines to Know
- Fall Season Sports: Physicals and FinalForms Submissions are due no later than Wednesday July 1, 2026.
- Winter and Spring Season Sports: Physicals and FinalForms Submissions are due no later than Tuesday, August 11, 2026.

Athletic Offerings at CBHS
Students are able to compete in 16 different athletic programs at CBHS. Our Brother's Boys have teams for eleven TSSAA-approved programs at Varsity, Junior Varsity and Freshman levels. We also offer club sports that are competitive with other schools throughout the region and intramural sports for our recreational student-athletes.
