    All athletes are required to have a yearly physical that is current to participate with school activities.  You need to have this physical on file with me before you start with any practices, workouts, or competitions with your sport.  We have concluded our mass physicals for the high school with over 300 athletes attending.  If you do not attend the physical on May 12, 2010 you will need to:

    1. log on to the website listed, create an account
    2. answer the healthcare questionnaire
    3. print forms (there was some problems with printing but has been resolved)
    4. take to your doctor (primary care, walgreens, krogers etc) have your sport physical 
    5. have the doctor sign your printed forms
    6. return forms to Marty Scruggs before you participate

    Common Questions

    • My son is a freshman and we have obtained a physical in Feb. while submitting form to the school
      -- your physical is good for one year, you will need another in feb to keep him current, you still need to log on to the website, answer the questionnaire, print forms and just put the date of the physical on file in the blank that states he is cleared to participate without restrictions.
    • It wont let me print the forms
      --working with customer support, we have identified the problem and it should work now. 
      --it will not let you print forms until all questions have been answered.

    Insurance

    You are required to show proof of insurance before entering CBHS the sports programs do not carry insurance to cover your son. Some sports, such as football, have a catastrophic policy that will kick in after 10,000.00 of incurred expense. This is a program instituted by the TSSAA that the school purchases to assist. We are currently working on options that you can purchase as a secondary policy if you wish.

    NCAA Eligibility

    The National Collegiate Athletic Association governs over 900 schools in three divisions and sponsors championships in over 20 sports.  High school athletes who are being recruited by Division I or Division II colleges must meet the academic eligibility criteria of the NCAA.  During senior year of high school the athlete must request that his high school transcript be sent to the NCAA Initial-Eligibility Clearinghouse for evaluation.  The  Clearinghouse will provide this information to colleges requesting the student's eligibility status.  Visit the NCAA Eligibility Center  for more information and resources for College-Bound Student-Athletes and their parents.
